COMMUNITY DAY SERVICES AND VOLUNTEER/JOB DEVELOPMENT

Life Visions Provides a wide variety of meaningful activities throughout the day including; Skill building classes, theater classes, men’s group, women’s group, opportunities to work out at the gym and other activities that promote independence and a positive image within the community. We encourage each person to assist in developing an individualized schedule and we focus on the activities each person wants to participate in to achieve their goals. Life Visions provides vocational skills and volunteer training for individuals that have a desire to have a job in the community. We prepare people for work and volunteer expectations. Helping people make connections with coworkers and work with each individual providing job coaching and transportation according to people work schedule.

CONFLICT-FREE CHOICE CASE MANAGMENT

Life Visions has been committed since 2007 to providing quality and supportive Case Management. We are a team of seasoned Case Managers who coordinate and oversee services for individuals over 21 with Intellectual & Developmental Disabilities or Acquired Brain Disorders. Life Visions external Case Management is considered Conflict-Free (CFCM). CFCM ensures that the coordination of services is separate from the delivery entity of services. External Case Management provides the opportunity for choice and independence to limit any conscious or unconscious biases. Life Visions is a vendor agency that has the unique ability to understand the delivery side of services.

RESIDENTIAL SUPPORT

Life Visions matches the support needs and interests of each individual served with experienced home care providers. Families and individuals have the opportunity to meet potential providers to ensure the best match possible.

INDEPENDENT LIVING SUPPORT

Life Visions provides supports to individuals that live independently in a variety of environments. Support hours are scheduled according to the needs of each person on an individualized basis. We assist with budgeting needs, medical appointments, medication oversight, grocery shopping and transportation.

FAMILY DIRECTED SERVICES

Life Visions provides supports for adults and children that live with their families. We work with each individual, their family and community agencies to provide supports with developing independent living skills and social connections, accessing community resources and participating in recreational activities so each person can become active contributing members of their family and communities.
